- Birth, marriage and death certificates for every ancestor in the direct line
- US vital records from each relevant state
- Apostille from each state's Secretary of State
- Certified Italian translations of all US documents
- Italian ancestor's birth certificate from the original comune
- Certificate of non-renunciation of Italian citizenship
